OT: Resources for Software Design Documents (SDD)

Sorry to post an off topic subject, but I know there are a lot of smart people here who could help me out.

Can anyone recommend a good website or book that deals with software documentation?  I'm trying to learn the process for creating a good/useful SDD, but have not been successful.

Any tips, examples, links highly appreciated.
